MEMPHIS WEATHERIZATION PROGRAM
APPLICATION SUPPORTING DOCUMENTS

In order to expediate the process, please make sure
you have the following available when you submit your application:

 

IDENTITY

• Driver’s License
• Military I.D.
• Social Security Card
  

 

HOME OWNERSHIP INFO

• Mortgage statement
• Deed
• Other proof of homeownership
  

 

INCOME INFORMATION (Provide 3 months of proof of income)

• Pay Stubs
• Social Security Payment Information
• Disability Payments
• TANF Program Payment Information
  

 

MLGW INFORMATION

• 12 required with the minimum of 3 months of MLGW Bills
• Signed Energy Bill Waiver with MLGW account number
